Notice of Intention to Leave
If your child will be leaving Bethania Lutheran School and you wish to cancel their enrolment, families are required to complete a Notice of Leave form. This allows us to update our records and provide a smooth transition for your child.
How to Submit the Cancellation Form:
1. Access the Form

2. Complete the Required Details
Fill in the form with your child’s name, year level, intended final date of attendance, and the reason for cancelling enrolment.
3. Submit the Form
Once all information is entered, click Submit at the bottom of the form. A confirmation message will appear once your form has been successfully submitted.
4. School Follow-Up
Our team will review your submission and be in touch if any additional information is required. This also allows us to prepare any relevant documentation and ensure a positive farewell experience for your child.
